And when we don’t reach these goals, we become frustrated and throw in the towel.  But the good news is that it doesn’t have to be this hard.  There are three steps that you can take to look your very best but you need to be compliant.  Simply attend to one, and you simply won’t accomplish your goals.</p>
<p>But before we discuss these steps, I encourage you to explore your mindset going into this.  Identify not only what you are trying to accomplish but also why you are trying to accomplish this specific goal now.  And, what are you willing to give up to actually accomplish it?  By examining your motivations for any resolution or new path before moving forward you can often avoid common missteps and achieve more long-lasting success.  </p>
<p>With that in mind, let’s talk about the three steps you absolutely need to follow to achieve your best looking body ever!</p>
<p>First and foremost, you need to eat right.  Any personal trainer, nutritionist, or dietician will tell you that what you eat contributes as much as 80% to how well you look and well how feel overall!  That’s a pretty incredible number.  Think about it.  How many of your friends (or even you) have worked out non-stop, gone to the gym continuously, but still have not accomplished their goals.  If you really look at what they (and you) are eating, many people use exercise as an excuse to eat whatever they want.  The mindset is that you exercise so that you can eat but this mindset is a trap and is one thing that can keep you from your goals.  Diet is critical and needs to be the foundation of your resolution.  And when I say diet, I don’t mean a diet that is so restrictive you can never maintain it; I mean a plan for eating that is rigid enough to keep you on focus but flexible enough that you can actually stick with it.  </p>
<p>Next, you need to exercise.  Simply eating right will never get you optimal results.  You need to exercise on a regular basis.  And, as recent studies demonstrate, you need to raise your heart rate and you need to keep it fun.  The more you enjoy how you exercise, the more likely you are to stick with it.  And, as we all know, that is really half the battle.  Many people join a gym as a New Year’s resolution only to fade out early.  Instead, create habits that are healthy, that are fun, and that can be embraced on a long term basis.  If you have a dog, run with your dog and use that to supplement your physical activity.  If you enjoy the outdoors, find a local hike that will raise your heart rate and allow you to enjoy the natural beauty around you.  Whatever you do, find something that you can stick with and that will give you enough exertion but enough enjoyment that you will actually want to exercise on a routine basis.  </p>
<p>Finally, when all else fails and those problem areas simply won’t go away, surgery is always an option.  And since your ultimate outcome will only be as good as the person behind the procedure, it is critical that you research both the background and experience of anyone you are looking to for liposuction.    Look for a Board Certified Plastic Surgeon who is experienced in body contouring, ask to see photos of their results, and (as importantly) ask to speak with one or two of their clients.  This last recommendation is especially important because it may give you even more insight into not only the caliber of results you can expect but also the experience you will encounter along the way.  Over the last several years and as a result of economic pressures within the medical industry, many non-core physicians now offer liposuction and other body contouring procedures to make ends meet.  And they often do so at bargain pricing to entice clients and grow their patient base.  Is this always a bad thing?  Not always.  But keep in mind that many of these physicians have never even completed a surgical residency and are suddenly performing what is truly a surgical procedure.  Or, if they do have surgical experience, it is not in this area.  Think about it, do you really want a Family Practitioner whose experience is limited to treating coughs, colds, and long term non-surgical conditions to be performing your surgery?  In my book, that is simply not good medicine.  And it certainly should be a red flag for you as the consumer.  </p>
<p>With these three suggestions in mind, I wish you the very best in the New Year and encourage you to embrace change as a long term solution keeping in mind that most good things come with compromise and persistence as well as a change in the way you see the world around you.  Be well!</p>

This new kid on the block goes by the name Xeomin and is brought to you by the same company that manufactures the filler injectable, Radiesse.  So what’s the big deal and will Xeomin be any better than it’s currently available alternatives Botox and Dysport?</p>
<p>All neuromodulators, such as Botox, share a common core protein.  This single protein is responsible for blocking the message from the nerve telling the muscle to fire.  The result is that the muscle does not receive the message; the muscle doesn’t contract, and over time the skin overlying this muscle relaxes and its wrinkles soften and often go away completely.  What differentiate the various products are the additional proteins surrounding this central core.  These accessory proteins affect specific qualities of the injectable such as how many days it requires to take effect, how long it lasts, and it’s potential for local diffusion.  And this is where Xeomin is so different.  </p>
<p>Xeomin varies from all currently available products in that it is a “naked” toxin.  While it contains the same central core protein, it brings with it no accessory proteins.  What’s the big deal, you ask?  Each time we are exposed to proteins, our bodies potentially can react to these specific proteins and develop antibodies.  A good example of this is when we get a flu shot.  In the case of the flu, it’s great that we develop antibodies.  But in the case of a neuromodulator, when we develop antibodies we effectively develop immunity and so the product becomes less and less effective until it simply doesn’t work.  And for that reason investigators are looking into whether or not Xeomin may be a more effective option in the long run than either Botox or Dysport.  So far, this has not been proven in the lab nor has it been seen among European injectors who have had access to all three products for several years prior to their approval in the states.  As such, only time will tell.</p>
<p>Xeomin will most likely be priced similar to its counterparts and so don’t look for tremendous cost savings.  Like any injectable, many practitioners will probably offer incentives early on to introduce the new product to their patients but, in the long run, treatment costs will most likely even out.  </p>
<p>As with any injectable, results are only as good as the person behind the needle.  Look for a well-trained professional specifically skilled in this area that listens to your needs but is honest about anticipated outcome.  As a national trainer, I have seen price points all across the board for these products and should emphasize that price should always be the last determining factor in who performs your injections.  Remember, these results can last several months and achieve dramatic improvements in facial aging when performed correctly.  The right product in the right hands is the best equation for a good outcome.  </p>

			<content:encoded><![CDATA[<p><a href="http://www.beautybybuford.com/wp-content/uploads/2011/07/Calmplex.jpg" class="fancybox"><img class="img-left" style="padding: 5px;" src="http://www.beautybybuford.com/wp-content/uploads/2011/07/Calmplex-300x225.jpg" alt="" width="300" height="225" /></a> A recent study in Dermatology Times remarked that people with Rosacea say the disease can negatively affect their workplace interactions and employment — even to the extent of being denied promotions.  In this survey of 1134 Rosacea patients by the National Rosacea Society, 60 percent of all people responding and 88 percent of those who reported severe symptoms said that the disorder had negatively impacted their workplace interactions.  And among those who reported severe symptoms, 51 percent said they had actually missed work because of their condition. </p>
<p>Rosacea is a very common disorder resulting in redness and flushing to the face.  Most patients begin with a baseline of increased redness but may experience pronounced flushing when exposed to stimulating agents such as heat, alcohol, or even stress.  And while there is really no cure for Rosacea, the good news is that its symptoms can generally be treated very easily with minimal downtime. </p>
<p>The initial starting point for treatment of Rosacea is good skincare.  And while effective anti-aging products and ingredients can be used, there should be a focus on gentleness since even the slightest amount of irritation can often produce dramatic flaring in a patient with Rosacea.  For our patients, we individualize a program that is gentle and something that will not irritate their skin long-term.  In addition, we have found good success with the SkinMedica CalmPlex product.  When used for at least 90 days, this topical has shown very good success at taming redness and inflammation with very little downside.  Since it is emollient, we encourage our patients to use a small amount of product and increase up as tolerated. </p>
<p>In addition to good skincare, one of the staples of treating Rosacea is broadband light therapy.  This procedure (BBL) involves a few treatment sessions but is associated with minimal discomfort and almost no downtime. </p>

		<title>Training Is Everything: A Must Read Before You Move Forward With Your Plastic Surgery</title>

			<content:encoded><![CDATA[<p><img class="img-left" style="padding: 5px;" src="http://www.beautybybuford.com/wp-content/uploads/2011/07/21391s4dcamelk4-199x300.jpg" alt="" width="99" height="200" /></a>On a sad note, a news update detailed the recent suicide of a General Practitioner in South Africa following his investigation for several botched cosmetic surgeries.  Dr. Christiaan Botha took his own life less than a month before he was scheduled to appear before the Health Professions Council of South Africa (HPCSA) on charges relating to practicing medicine outside the scope of his qualifications. </p>
<p>According to the article, Dr. Botha received his medical degree from the University of Pretoria and then ultimately was trained as a General Practitioner.  He then began performing cosmetic surgery after 10 years of “self-study” and assisting qualified surgeons.  The most recent hearing was related to six new cases of women for whom Dr. Botha performed breast augmentation but who all had developed surgical complications following their procedures. </p>
<p>The HPCSA accused Dr. Botha of failing and neglecting to provide proper care to these patients once they developed surgical complications.  Overall, there were actually an additional two cases of suspected negligence but Dr. Botha apparently paid an “admission of guilt” fine for the other when he appeared before a prior committee of inquiry. </p>
<p>Dr. Botha’s attorney recently won an appeal for the good doctor who was sentenced for another botched operation in 2007 in which the patient’s nipple apparently fell off her breast.  Subsequently, she developed infection to that breast and required extensive removal of tissue.  And a week after being prosecuted for this patient’s outcome, Dr. Botha was found guilty of the same offense to another patient.  He was ultimately suspended for a year, nine months of which was suspended for three years. </p>
<p>Following the initial sentence, Dr. Botha was found guilty of unprofessional conduct and was ordered to close his cosmetic surgery practice and to stop practicing as a GP for a year. Interestingly, that sentence (to stop practicing as a GP for a year) was ultimately suspended for a year. However, Botha appealed the sentence and won in March this year. The sentence was changed to a five-year sentence wholly suspended for 10 years.</p>
<p>While this story is obviously sad on multiple levels, everything could have been prevented.  And, unfortunately, this type of story is becoming more and more common as unscrupulous physicians practice outside their scope of training and harm the very people they were trained to protect. </p>
<p>As a Board Certified Plastic Surgeon, I have seen this trend develop and grow on not only a national but an international level.  And while we are all human and we all are prone to mistakes, we are also educated enough to know that the MD degree does not guarantee outcomes nor does it allow us to practice whatever we want. </p>
<p>To protect yourself, you should always know the breadth and experience of training that your medical professional has before undergoing any procedure (no matter how benign).  A few well-defined questions may mean the different between a successful outcome and years of complicated revisions.  I encourage all of my patients to ask at least the following basic questions:</p>
<ul>
<li>How long have you been practicing medicine?</li>
<li>What specialty is your training in?</li>
<li>How many of these specific procedures have you performed?</li>
<li>What is the worse outcome of this procedure that you have seen?  How did you fix the outcome?</li>
<li>What are my options if I am not satisfied with my result?</li>
<li>Can I see before &amp; after photos of this procedure?</li>
<li>Can I speak with a patient of yours who has undergone this procedure?</li>
</ul>
<p>Again, these are very basic questions and should not replace your gut instinct.  They should merely augment your comfort level when you do choose a qualified surgeon. </p>

			<content:encoded><![CDATA[<p><img class="img-left" style="padding: 5px; title="botox-199x300" src="http://www.beautybybuford.com/wp-content/uploads/2011/07/botox-199x300.jpg" alt="" width="199" height="300" /></a>I recently participated in an Advisory Board Panel Discussion on <a href="http://www.beautybybuford.com/face/botox-denver-co/">Botox®</a>.  As always, the conversation was lively and there were a number of very valuable insights that I learned from interacting with my Plastic Surgery and Dermatology peers. </p>
<p>First, we talked about the relative growth of Botox® over the last few years and how this treatment has grown in popularity, despite the economic downturn, and effectively out-performed many of the other elective procedures that we are all performing in our offices.  That being said, there was a common feeling that while a majority of our Botox® patients previously saw it as a “nice to have”, they now see their treatments as a “must have” and will actually budget and allot dollars towards each session.   In fact, some practitioners even went so far as to say that their clients saw it as their edge in a workplace that has grown increasingly competitive and that they wouldn’t think of giving it up.  And from the sentiments of my clients, I can certainly support that.</p>
<p>Next, we talked about trends in the number of units and treatment areas that I clients are choosing.  The overwhelming sentiment was that a glassy, polished forehead is no longer the norm and that more and more clients are opting for fewer units in this area to allow for maximal lifting of the brows.  What many of us have found is that the more aggressively we treat the forehead muscle and the more we target these lines, the less lifting and shaping we have of the lateral brows.  Think of this way.  The face is essentially comprised of muscles that lift and muscles that depress.  To reverse aging, we generally try and treat the muscles that pull tissue down to allow for maximal lift.  However, given that the forehead muscle is a “lifter”, the more we treat it the less arching we see of the brows.  So many of us have opted for a more conservative approach to the treatment of the forehead and explain to our patients that they can either have fewer lines and wrinkles or more optimal shaping of the brows.  For the most part, my clients generally prefer the latter.</p>
<p>In contrast, the central brow (or “glabella”) appears to be an area where most of our clients still prefer a stronger treatment.  The more we furrow our inner brows over time, the more chance we have of developing the notorious “11’s”.  And when we do, we can look angry, mean, or simply upset when in reality we are none of these.  And so the treatment pattern here is still focused on a stronger blockade of the brow muscles along with gentle elevation and shaping of the inner brow. </p>
<p>The crow’s feet area is less well-defined.  Some clients like a stronger treatment and a more thorough eradication of these lines while some prefer to maintain these lines of expression and go for a more subtle approach.  As with all of the other areas, discussion with our clients before treatment is essential in determining what should be treated and how aggressively it should be managed. </p>

			<content:encoded><![CDATA[<p><img class="img-left" style="padding: 5px;" title="weightloss" src="http://www.beautybybuford.com/wp-content/uploads/2011/07/weightloss-300x274.jpg" alt="" width="300" height="274" /></a>If you are overweight and considering elective Cosmetic Surgery, you might want to shed a few pounds before moving forward.  A recent Johns Hopkins study by Dr. Martin Makary found that obesity was a factor that increases the risk for complications among obese patients undergoing elective breast procedures.</p>
<p>The study examined patients who underwent elective breast surgery between 2002 and 2006 and included 2403 women in the obese group and 5597 in the normal-weight group.  They found that within the obese groups, 18.3% experienced complications within 30 days of surgery compared with a mere 2.2% rate of complications in those of normal-weight.   Interestingly, differences between the two groups was greatest in cases of inflammation where obese patients were noted to be 22 times as likely to suffer a complication as compared to those determined to be of normal weight.  They were also 13 times as likely to experience infection and 11 times as likely to report pain.</p>
<p>Improvements in both surgical technique and anesthesia in addition to refinements in peri-operative care have made elective procedures much safer than ever before.  That being said, there are still factors which will dramatically increase your risk for complications both in the early as well as in the late healing period.  To optimize your results and reduce your risk for untoward effects, factors such as obesity need to be taken into account to ensure both a smooth surgery as well as an uneventful recovery.</p>
<p>In my practice, I counsel patients on factors which may potentially have a negative impact on their outcome and do whatever I can to try and address them.  On that note, Colorado was just rated as having one of the lowest rates of obesity and so this is not a risk factor that I encounter very frequently—especially with <a href="http://www.beautybybuford.com/breast/augmentation-denver-co/">breast augmentation</a> or <a href="http://www.beautybybuford.com/breast/lift-denver-co/">breast lift</a> procedures.  Instead, I usually see patients for body contouring who would benefit from losing a few pounds before their surgery in order to achieve the most optimal results.  In my opinion, if you are more than 30-40 pounds over your ideal body weight, unless there are other mitigating factors, you really should try and lose that weight before moving forward with surgery.  In addition to an increase in risk factors, as we have seen with this study, there is also a refined ability to create better contour when a patient is closer to their ideal body mass. </p>
<p>That being said, I recently saw a patient who is not close to her ideal body weight but who had a significant amount of extra abdominal skin and was having trouble with chafing and rashes.  I suggested to her that we move forward with a <a href="http://www.beautybybuford.com/body/tummy-tuck-denver-co/">tummy tuck</a> (to tighten the muscle and remove the excess hanging skin) and then look at possible <a href="http://www.beautybybuford.com/body/liposuction-denver-co/">liposuction</a> and additional tightening down the road.  In this case, a staged procedure is acceptable and the risks warranted. </p>
<p>Regardless of the procedure you are interested in, always consider a few things before moving forward.  First, make sure that you are prepared both physically and mentally for the surgery and the recovery and that you are doing it for the right reason.  Next, make sure that the surgeon you choose not only knows what they are doing but is prepared if everything doesn’t turn out exactly as planned.  Since there are no guarantees in life, this last point is even more important and so be sure and evaluate your potential Plastic Surgeon’s training, certification, and experience.  In the end, this makes all the difference in the world.</p>

<p>A recent article by Dr. Laurie Casas of Northwestern University School of Medicine discussed the challenges of a <a href="http://www.beautybybuford.com/breast/enhancement-denver-co/">breast augmentation mastopexy combination procedure</a> and stressed the importance of a thorough consultation before surgery that includes a detailed discussion of the anticipated outcome as well as the more common concerns associated with this challenging procedure.  While she emphasized the need for open communication, she ultimately commented that this procedure, when performed correctly, can provide a very high level of satisfaction for women who have lost breast volume either through the aging process, pregnancy, or as the result of significant weight loss.</p>
<p>Over the years, I have come to appreciate the challenges of the augmentation mastopexy procedure and have made a number of fine adjustments to increase the likelihood of a good outcome and happy patient.  In my practice, a bulk of the women that I see have lost breast volume and shape as the result of pregnancy-related changes to their breasts.  As a result, they are looking not only to address the concern of inadequate volume, but they are also looking to regain a more youthful shape overall.<br />
 <br />
In my opinion, to achieve the most optimal result, there needs to be a very careful and comprehensive discussion with each patient prior to moving forward to discuss not only what can be changed but also what cannot.  Pre-operative expectations are critical in that they can single handedly determine the success of the procedure as they are the determining factor in which each person expects they will actually see after surgery.  Associated scarring should also be a part of this discussion since this is one of the major reasons for a patient to not move-forward with surgery.  To address this concern, I am now performing complimentary laser scar resurfacing for my patients as early as three weeks following surgery and am seeing a very nice softening of the scarring long-term. <br />
During the initial consultation, I review with each patient the local anatomy and physiology of their breasts and explain what really happens as the result of the aging process.  In addition to changes in the breast volume itself, there are also changes that occur to the structural framework (“Cooper’s Ligaments”) which create an overall laxity of the supporting tissue and which, in reality, are never really addressed in any breast lifting procedure.  Because of this, I personally have reduced the overall size of implants used with procedure because  I feel that by using larger, heavier implants, I am simply adding additional weight to the breasts and potentially contributing to (and possibly accelerating) the gradual process of descent.<br />
 <br />
A lift itself is really only a skin and glandular shaping procedure and does nothing to tighten the internal supporting ligaments.   Because of this, I am careful to discuss expectations related to the degree of anticipated “perkiness” actually attainable after this procedure.  If a woman expects to have breasts of a 16 year old following surgery, she is doomed to failure because this type of result is simply not attainable.  The patient with realistic expectations understands that her breasts will be fuller and more shapely but that the pre-existing degree of internal laxity will still remain even following surgery.  That being said, the <a href="http://www.beautybybuford.com/breast/enhancement-denver-co/">combination of breast augmentation and breast lift</a> (“Augmentation Mastopexy”) when performed by an <a href="http://www.beautybybuford.com/about/choosing-a-surgeon/">experienced Plastic Surgeon</a> with expertise in cosmetic breast surgery, can achieve results that are very dramatic and that can have positive dramatic effects on a woman’s body image.</p>

<p>As Spring rapidly approaches, so also do reunions, weddings, and all of the photography that go along with these events.  But for many people, photos are simply no fun because they simply do not like their facial profile and the seemingly benign act of taking a candid shot can be painfully uncomfortable.  As we age and as we gain and lose weight, many of us develop increasing fullness to the neck just beneath the chin.  This “waddle”, as many patients refer to it, can become distressing and make us appear older than we really are.  And for many people, simply losing weight will not do the trick.<br />
 <br />
Over the last several years, I have seen a number of patients benefit from a straightforward combination procedure that effectively has taken years off their appearance.  This procedure combines <a href="http://www.beautybybuford.com/body/liposuction-denver-co/">ultrasonic VASER liposuction</a> with a chin implant and create dramatic results in less than an hour and with minimal downtime and discomfort. <br />
 <br />
To better understand why and how this combination is so effective, let’s consider who is a good candidate.   First, the ideal candidate is relatively young (under the age of 50) and has a moderate degree of fullness to their submentum (area beneath the chin).  This fullness makes the angle between the chin and the neck (“cervicomental angle”) obtuse or more open and creates a more aged appearance overall.<br />
  <br />
Next, many of these patients have what we refer to as a “recessed” chin.  This means that if we drew a line from their top lip to their lower lip and then to their chin, the chin would sit behind this line.  When this happens, a recessed chin makes the distance between the angle of their jaw and their chin point look short.<br />
 <br />
To address these issues, the following steps are performed.  First, a very small 1.5-2 mm access incision is made just beneath your chin.  Through this incision, a small amount of wetting solution is instilled just beneath the skin.  This solution provides numbing relief and also contains epinephrine which not only makes discomfort minimal but dramatically reduces the risk for bruising.  The solution is allowed to take effect for about 10 minutes following which ultrasonic energy is applied to the tissues beneath the skin.  This energy not only allows for melting of fat but also may have the potential to effectively tighten the neck skin.  In addition, the skin is effectively separated from the fat and muscle and a new plane created.  Once this is accomplished, gentle suction is then applied and fat is removed.  The result?  A smoother, more youthful neckline.  But wait…there’s more!</p>
<p>Once the central triangle of the upper neck has been addressed, the chin is then readied.  To do this, the lower lip is numbed and a small incision made inside the mouth on the lower lip through which the bony chin is reached.  Chin sizers are then placed to evaluate an appropriate degree of augmentation.  Once an appropriate size has been decided, the tissue pocket is cleansed with antibiotic solution and the permanent implant placed.  Tissue layers are then closed over the implant holding it in place and taping is used externally to fix the implant in place.  For many patients, a small amount of <a href="http://www.beautybybuford.com/face/botox-denver-co/">BOTOX®</a> can be injected a few days prior to surgery to help reduce muscular movement of the chin in the early healing period and potentially allow for more adequate stabilization.<br />
 <br />
The final step involves placement of a supportive head-wrap which remains in place for 5 days.  The wrap applies pressure to the neck and dramatically reduces swelling and bruising and helps the skin better adhere to the underlying tissues.  Once removed, a chin strap is then work at night for the next two weeks.  During this time, it is recommended to avoid sleeping on pillows and flexing the neck.<br />
 <br />
And the end result?  A more youthful neck and a more structured and well-balanced jawline.  While many of these procedures are performed under general anesthesia, they can very easily be performed using light sedation with similar results. </p>

<p><strong>How should you prepare for you SkinTyte treatment?<br />
</strong>Prior to your appointment, all facial make-up, ointments or creams must be removed. Men will be asked to shave on the day of treatment prior to arrival at the office.<br />
Before starting your treatment, eye shields or goggles will be placed to protect your eyes.  And for those with very sensitive skin, a numbing gel may be placed for up to 45 minutes before treatment to keep you as comfortable as possible. </p>

<p>For optimal results (and depending upon body areas treated), most patients require anywhere from 2-5 sessions with each session spaced 3-4 weeks apart with sessions generally lasting around 45 minutes depending upon the number of areas treated. </p>
<p><strong>What is involved with the treatments?</strong><br />
Because our goal is to keep you as comfortable as possible, we may suggest that a topical numbing cream is used prior to your procedure.  And for those more sensitive patients, we can prescribe a mild medication for pain and/or anxiety. </p>
<p>Expect some degree of minimal facial swelling the day following your treatment. This will resolve pretty quickly and most patients are able to wear makeup soon after. Keep in mind that it takes time for changes to the collagen to occur and so maximal results are generally seen around 3-6 months after your initial visit. </p>
<p>Post-treatment pigmentation has been noted on occasion, but is highly treatable and fortunately very rare. Even rarer is a decrease in pigmentation of the skin after treatments. This also clears in time as well. We have never seen a scar or infection of the skin after treatment but it can occur. Swelling of the face for 3-5 days occasionally occurs and is most common in severely sun damaged skin due to reduced quality of connective tissue around the blood vessels.</p>
<p>After an initial series of 3-5 treatments, most patients achieve maximal continued results through maintenance sessions. In addition, if you are not currently on a medical skin care regimen, our skilled aestheticians will personally design one for you that will help protect your skin for years to come. </p>
